Ανακοινώνεται ότι για τις εγγραφές στον Παιδικό Σταθμό του Δήμου Κυθήρων για το Σχολικό Έτος 2024-2025 θα πρέπει να υποβληθούν τα παρακάτω δικαιολογητικά:
1. Application to register a child at the Childcare Centre
2. Πιστοποιητικό οικογενειακής κατάστασης, όπου θα εμφανίζεται απαραιτήτως το παιδί, για το οποίο γίνεται η αίτηση και φυσικά θα επιβεβαιώνεται η κατάσταση της οικογένειας(πολύτεκνη οικογένεια, τρίτεκνη οικογένεια, μονογονεϊκή οικογένεια, ορφανική οικογένεια). Εάν δεν είναι στην Ελληνική γλώσσα θα πρέπει, να συνοδεύεται και από επίσημη μετάφραση.
If there is a cohabitation agreement, it must be presented, as well as the tax returns of both parties.
3. Αντίγραφο εκκαθαριστικού σημειώματος:
– Αντίγραφο εκκαθαριστικού σημειώματος του τρέχοντος φορολογικού Έτους ή
– Αντίγραφο εκκαθαριστικού σημειώματος του προηγούμενου φορολογικού Έτους , σε περίπτωση που δεν έχει εκδοθεί μέχρι την ημερομηνία κατάθεσης της αίτησης ή Υπεύθυνη Δήλωση (το έντυπο της δήλωσης χορηγείται από την Υπηρεσία ή από την ιστοσελίδα του Ν.Π.Δ.Δ.), προσκόμισης εκκαθαριστικού σημειώματος του τρέχοντος έτους
4. Κατάσταση υγείας του παιδιού, συμπληρωμένη απαραιτήτως από παιδίατρο. (το έντυπο χορηγείται από την Υπηρεσία) και φ/α του βιβλιαρίου Υγείας του παιδιού.
5. ΔΙΚΑΙΟΛΟΓΗΤΙΚΑ ΕΡΓΑΣΙΑΣ (ανά περίπτωση): (αφορούν και τους 2 γονείς προκειμένου να µοριοδοτηθούν)
A) FOR WORKERS IN THE PRIVATE SECTOR:
A certificate of employment from the employer, dated no more than two (2) months from the date of commencement of registration applications.
Copy of stamps printed by IKA (last quarter) or Ego stamp (copy of a stamp and proof of deposit in a bank in the last year before the submission of the application, proving at least five (5) months of work).
B) FOR EMPLOYEES IN THE PUBLIC SECTOR/LOCAL GOVERNMENT OR THEIR PUBLIC SERVICE BODIES:
A certificate of employment from the relevant institution not older than two (2) months from the date of the start of the registration application.
C) FOR INDEPENDENT PROFESSIONALS:
Copy of the Business Start Declaration to the Tax Office
Responsible declaration of Law 1599/86 on the non-interruption of the exercise of the business.
D) FOR COMPANIES IN CORPORATION or S.A. or SINGLE-PARTY LTD:
Articles of Association and any amendments thereto.
Responsible statement of Law 1599/86 on the non-interruption of the exercise of the business.
(E) FOR SELF-EMPLOYED PERSONS IN THE PRIMARY SECTOR:
Certificate of insurance from the O.G.A. or certificate from the O.G.A. that insurance contributions have been paid or adjusted
F) FOR CASES OF UNEMPLOYED PARENTS:
Unemployment certificate , not older than ten (10) days from the date of the start of registration applications, or a valid unemployment card, of the parent or parents who are unemployed.
SPECIFIC SUPPORTING DOCUMENTS PER CASE:
i) In the case of divorced parents, a copy of the divorce decree is required (if this is not mentioned in the certificate of marital status).
ii) In the case of separated parents, the following is required:
-Copy of the divorce petition (if there is one)
-Confirmation of change of individual data of the Tax Office, proving the separation (if any).
-Court decision, which will indicate the custody of the child (if there is one) or a Declaration of Responsibility of Law 1599/86 in case the above have not been issued
(The above documents must be dated before the start of the registration period).
*In the case of divorced or separated parents, in order to be considered for the job, the documents of both parents must be presented. Otherwise, only the person who has custody of the child will be awarded a credit.
-Certificate of death of the deceased parent (for an orphaned child), if this is not mentioned in the certificate of family status.
Για την εγγραφή παιδιού αλλοδαπών γονέων:
-Copy of a valid Identity Card or Passport, necessarily with an official translation (if not in Latin script).
-Legal residence permit or application for legal residence in the country, as evidenced from time to time.
*Certificate from the Secretariat of the School of the last semester (for parents who are students or students of the first cycle of studies).Certificate from the Army (for parents of soldiers).
*In the case of parents with a child belonging to the disability group with disability rate 67% or a parent belonging to the disability group with disability rate 67%, a certificate from the Health Committee or a certificate from a public hospital (valid) is required.
All the above documents are required for re-registration. If there are no changes in family status, a solemn declaration by the parents confirming that there have been no changes is required.
All children who have reached the required age of 2.5 years on the date of the opening of the day care centre have the right to register.
Οι αιτήσεις εγγραφών/επανεγγραφών για το Παιδικό Σταθμό του Δήμου Κυθήρων για το έτος 2024-2025 θα ξεκινήσουν 01 Ιουνίου και θα διαρκέσουν έως 30 Ιουνίου .
Οι γονείς /κηδεμόνες μπορούν να υποβάλουν την αίτηση ηλεκτρονικά στο email: στο kythira@otenet.gr έως τις 30 Ιουνίου 2024.
